Administrative Reporting Guide
       Use this guide as a quick reference for your administrative reporting obligations as a contractor with BCOPD.  If anything here is unclear or conflicts with your professional services agreement, your agreement will control.

FA-3 Claim for Compensation (Legal Services)

  Use this form to claim compensation for legal services you provide on a one-time or hourly basis.
FA-3J Claim for Compensation (Legal Services Juvenile Case)

  Use this form to claim compensation for legal services you provide on a one-time or hourly basis in juvenile cases.
FA-3R Claim for Compensation (Legal Services Recurring)

  Use this form to claim compensation for legal services you provide on a recurring basis pursuant to any contract where you are paid for cases on a per-case basis.
FA-4S Travel Expense Supplemental Claim Form

  Use this form to claim reimbursement for expenses you incur in travel for public defense cases.
FA-5 Caseload Reporting Form
  Use this form to report your caseload for any monthly compensation or per-case compensation contract.  This form now incorporates the reporting of key motion and case disposition data to be collected and reported for Performance Metrics reporting purposes.

M-1 Annual Reporting of Private Practice Hours

  Use this form to provide your annual report of private practice hours as required by RCW 10.101 and your contract.
M-2 Annual Reporting of CLE Hours
  Use this form to provide your annual report of CLE hours as required by RCW 10.101 and your contract.

Contractor Payee Change Form

  Use this form to change the name to whom compensation checks are issued (for example when you form a business entity under which to practice law or you join a firm).

Contact Info Change Form
  Use this form to change your contractual contact info both for mailing of compensation checks and for contract notices.

Quarterly Certification for Attorneys   Use this form to certify compliance with Washington's Public Defense Standards as required by Supreme Court Rule.

Competency Evaluation Form Order   Use this form motion to seek a court order directing an independent competency evaluation after an initial competency evaluation has already been conducted at or by Eastern State Hospital.

Justice Center Background Check Form (Benton)

  To obtain a security clearance to access the Benton County Justice Center without going through security screening, complete this form in its entirety and return to BFOPD.

JIS-Link Application and Contract
  Use this form to set up an account with State OAC to access JIS-Link free of charge.  Note that the Public Defense Manager will have to sign the appropriate certification in the form confirming you are a public defender.

Sample Invoice   All compensation requested using a Claim for Compensation require an accompanying invoice.  If you don't have software that generates an invoice, this sample can be used with Excel to generate a compliant invoice.

SSOSA Evaluation Form Order
     Use this form motion to seek a court order finding a defendant presumptively qualified for a SSOSA sentence and directing an examination.

IRS W-9 form

  An IRS W-9 form is necessary to establish your business as a vendor for Benton County in order for you to be paid.  This only needs to be done if you haven't been paid as a vendor before.
